ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES It is ORDERED that Plaintiff cure the deficiencies designated above within thirty (30) days from the date of this order. Any papers that Plaintiff files in response to this order must include the civil action number on this order. Plaintiff shall obtain and utilize the District of Colorado's 1) Complaint Form (General Complaint), and 2) Application to Proceed in District Court Without Prepaying Fees and Costs (Long Form), along with the applicable instructions, available at www.cod.uscourts.gov. Plaintiff may pay the $405.00 filing fee instead of submitting the Application to Proceed in District Court Without Prepaying Fees and Costs (Long Form). Plaintiff fails to cure all of the designated deficiencies within thirty (30) days from the date of this order, the action will be dismissed without further notice. The dismissal shall be without prejudice. The Application to Proceed In Forma Pauperis (ECF No. 2) is denied without prejudice to being refiled on the correct form.
