Notice of Deficiency and Order. Plaintiff must address the following deficiency: the $350.00 filing fee and $50.00 administrative fee (total fees of $400.00) were not paid nor has a request to proceed in forma pauperis been submitted. Plaintiff must also submit a complete amended complaint on the court-approved form for civil-rights cases. Failure to file the required documents will result in a denial of in forma pauperis status and a dismissal of Plaintiff's complaint for want of prosecution without further notice, unless the filing fee of $400.00 is timely paid. Deadline to cure the deficiency is 20 days.
