Thus, the Court finds that Plaintiff shall have 20 days from the date of this order to file a complete amended complaint on the proper form and to pay the filing fee of $402.00 or to file the following documents within 20 days from the date of this order, a signed application to proceed in forma pauperis; and a certified copy of his inmate trust account statement.
