Petitioner must (1) file his petition on the court-approved form, and (2) either pay the $5.00 filing fee or file an application to proceed in forma pauperis with a certificate of his inmate trust account, within 30 days from the date of this order. Additionally, the Court notes that although Petitioner signed his letter, it also bears the signature of his family member. Petitioner submitted a document titled "Statutory Durable Power of Attorney" with his letter. To the extent that Petitioner intends for his relative to represent him in this matter, he cannot do so. The Clerk is directed to change the caption to reflect that the proper Respondent is the Director of the Texas Department of Criminal Justice, Correctional Institutions Division (TDCJ-CID).
