ELECTRONIC DEFICIENCY SHOW CAUSE ORDER: On April 14, 2026, the Court ordered Petitioner to file an amended petition on the proper form and to pay the filing fee of $5.00, or, if he is unable to pay, to file a complete, signed application to proceed in forma pauperis and a certified statement of his inmate trust account. Dkt. No. 4 . Petitioner filed his amended complaint and application to proceed in forma pauperis, but the inmate trust account statement does not appear to be certified by an authentic signature of an officer at the unit where he is incarcerated. See Dkt. No. #7 at 3. The Court therefore orders that Petitioner must, no later than June 12, 2026, (14 days), file a certified statement of his inmate trust account, or show cause why his petition should not be dismissed for want of prosecution pursuant to Fed. R. Civ. P. 41(b) because of his failure to respond to the Court's order. Petitioner's failure to comply with this order will result in the dismissal of this action pursuant to Federal Rule of Civil Procedure 41(b). Petitioner did not file the petition on the appropriate form. Nor did he pay the required $5.00 filing fee or submit a proper request to proceed in forma pauperis (IFP) along with a certified statement of his inmate trust account. See 28 U.S.C. 1915(a)(2). Therefore, Petitioner is ORDERED (1) to complete and return the enclosed form used in this Court for actions filed under 28 U.S.C. 2241, and (2) to pay the $5.00 filing fee or submit an IFP application with the required certificate of inmate trust account, by May 14, 2026 (30 days).
