Plaintiff Zseron Dukes must address the following deficiency: The complaint was not filed on the appropriate form and neither the $400 filing fee nor a request to proceed in forma pauperis was submitted. Therefore, by 9/3/2019, Plaintiff is ORDERED (1) to file his civil rights complaint on the enclosed form, and (2) to pay the $400 filing fee or file an application to proceed in forma pauperis on the enclosed form with the required certificate of inmate trust account. See Complaint form at page 2, paragraph 3 for filing fee information under the Prison Litigation Reform Act. PLAINTIFF'S FAILURE TO COMPLY WITH THIS ORDER MAY RESULT IN THE DISMISSAL OF THIS ACTION PURSUANT TO FEDERAL RULE OF CIVIL PROCEDURE 41(b).The Clerk of the Court is directed to provide to Plaintiff, with service of this Order, a prisoner civil rights complaint form and a prisoner form motion to proceed in forma pauperis.
