September 18, 2024 Opinion or Order Filing 9 ELECTRONIC ORDER AND NOTICE OF DEFICIENCY. Plaintiff Lazell D. Jackson, Sr., must address the following deficiency: The complaint and motion to proceed in forma pauperis were not filed on the appropriate form. See N.D. Tex. Misc. Ord. 14 (requiring inmates to file civil rights complaints and motions to proceed in forma pauperis on the court-approved forms). Therefore, Plaintiff is ORDERED to (1) file a civil rights complaint on the court-approved form, and (2) pay filing and administrative fees of $405.00 or file an application to proceed in forma pauperis on the court-approved form with the required certificate of inmate trust account, by October 21, 2024. See Complaint form at page 2, paragraph 3 for filing fee information under the Prison Litigation Reform Act. FAILURE TO COMPLY WITH THIS ORDER WILL RESULT IN THE DISMISSAL OF THIS ACTION PURSUANT TO FEDERAL RULE OF CIVIL PROCEDURE 41(b).The Clerk of the Court is directed to provide to Plaintiff, with service of this Order, a prisoner civil rights complaint form (labeled amended and reflecting this case number) and a prisoner form motion to proceed in forma pauperis. Any amended complaint will supersede (take the place of) the previous filing. Consequently, Plaintiff must state all claims and list all defendants in any amended complaint filed. Failure to provide updated address information may result in the dismissal of the case for failure to prosecute or follow court order under Fed. R. Civ. 41(b).
