Date Filed Document Text
May 22, 2026 Filing 5 MOTION for Leave to Proceed in forma pauperis filed by Carla Sue Ford. (axm)
May 14, 2026 Opinion or Order Filing 4 Order and Notice of Deficiency: Carla Sue Ford must address the following deficiency: Ford failed to either pay the filing fee ($405) or file a motion for leave to proceed in forma pauperis ("IFP"), the complaint fails to state a claim for relief. Section 1915(e)(2) requires that the Court "dismiss the case at any time" if it "fails to state a claim on which relief may be granted." 28 U.S.C. 1915(e)(2)(B)(ii), and the complaint is deficient because it does not provide sufficient information for the Court to determine whether venue is correct in this district because it does not contain any information as to the location where Ford was employed when the events underlying her claims occurred and is unclear as to whether Ford was working remotely or at a branch location of Prime Therapeutics. Ford must, by June 15, 2026, either file an IFP motion on the form provided with this order or pay the full filing fee of $405. Ford shall file any amended complaint by June 15, 2026. Failure to comply with this order may lead to dismissal for failure to prosecute pursuant to FRCvP 41(b). (Ordered by Magistrate Judge David L. Horan on 5/14/2026) (cea) (Additional attachment(s) added on 5/14/2026: #1 IFP) (cea).
