ELECTRONIC ORDER AND NOTICE OF DEFICIENCY. Plaintiff De'Malcolm D Hawkins must address the following deficiency: The complaint was not filed on the appropriate form and in compliance with Fed. R. Civ. P. 8(a). Also, the motion to proceed in forma pauperis does not provide enough information for the Court to determine if in forma pauperis status is appropriate. Plaintiff did not completely fill out the motion apart from listing zeros or writing N/A for most of the answers. Plaintiff also fails to specify who pays his living expenses. Further, a review of the docket reveals that Plaintiff has not yet registered as a user of the Court's electronic filing system, as required. Instructions for registering are found online at https://www.txnd.uscourts.gov/file-registration. Therefore, Plaintiff is ORDERED to (1) file an amended complaint on the form complaint for use in employment discrimination cases (Pro Se 7), (2) completely fill out the motion to proceed in forma pauperis on the AO-239 Form and resubmit it to the Court, and (3) register to use the Court's electronic filing system, all by September 16, 2026. FAILURE TO COMPLY WITH THIS ORDER WILL RESULT IN THE DISMISSAL OF THIS ACTION PURSUANT TO FEDERAL RULE OF CIVIL PROCEDURE 41(b). Both a form complaint for use in employment discrimination cases (Pro Se 7) and a form motion to proceed in forma pauperis (AO 239) are available online at http://www.uscourts.gov/forms/civil-forms. Any amended complaint will supersede (take the place of) Plaintiff's previous filing. Consequently, Plaintiff must state all claims and list all defendants in any amended complaint filed. ELECTRONIC NOTICE OF JUDICIAL SCREENING AND STANDING ORDER. Judicial screening of this case is pending. See 28 U.S.C. 1915(e)(2). It is therefore ORDERED that (1) service of process shall be withheld pending completion of judicial screening, (2) no motions for appointment of counsel shall be filed until the Court has completed the screening process, (3) all discovery in this case is stayed until the Court enters a scheduling order, (4) no amendments or supplements to the complaint shall be filed without prior Court approval, and (5) any motion to amend the complaint must be accompanied by a complete proposed amended complaint on the required form. It is further ORDERED that Plaintiff must promptly notify the Court in writing of any change of address by filing a written Notice of Change of Address with the Clerk of the Court. Failure to provide updated address information may result in the dismissal of the case for failure to prosecute or follow court order under Fed. R. Civ. 41(b).
