FILING FEE ORDER: Plaintiff is GRANTED leave to proceed in forma pauperis pursuant to 28 U.S.C. 1915. The agency having custody of plaintiff shall, when funds exist in plaintiff's inmate trust account or institutional equivalent, forward to the Court an initial partial filing fee of $75.00. Plaintiff shall pay $275.00, the balance of the filing fee, in monthly installments as provided in 28 U.S.C. 1915(b)(2). ORDER AND NOTICE OF DEFICIENCY: Iraida Hernandez must address the following deficiency: Although Plaintiff seeks to proceeds in-forma-pauperis, and has completed an in-forma-pauperis application, she has not provided a completed certified copy of a certificate of inmate trust account for the six month period immediately before filing the complaint. The Clerk shall mail to Plaintiff an Application to Proceed in Forma Pauperis, and a Certificate of Inmate Trust Account and Authorization, marked with the cause number for her use in complying with this Order. Deadline to cure the deficiency is 30 days. Failure to comply with this order may lead to dismissal for failure to prosecute pursuant to FRCvP 41(b).
