A certificate of inmate trust account (or institutional equivalent) for the six month period immediately before filing the complaint has not been submitted as required by 28 U.S.C. 1915(a)(2). A form application to proceed in forma pauperis (with cause number thereon) shall be mailed to Plaintiff along with a certificate of inmate trust account for completion and return to the Clerk of Court within the time period set forth below.
