ORDER: By an Order and Notice of Deficiency entered on September 13, 2021, the Court directed inmate/Plaintiff Eric Freeman to have a Certificate of Inmate Trust Account ("CTA") statement completed and filed within 30 days (or by no later than October 13, 2021) #5 . Freeman has filed a document in which he writes I did [what] they told me to request from [a] Bank Officer [and] Im indigent.... Response, 1, #6 . Freeman may be indigent, but the Court still requires him to have the CTA form completed and returned. The Court must review a completed CTA form to confirm whether Freeman is indigent, and to obtain the information required to impose the PLRA filing-fee collection.
