ECF No. #4 ; In order for the Court to make the determination regarding any pauper application, Plaintiff must provide complete and up-to-date accurate answers to all of the questions in the long form in forma pauperis application, including information about all of his income, assets, employment and banking deposits, and the income, assets and expenses of a spouse, if applicable.
