SECOND ORDER AND NOTICE OF DEFICIENCY: Secial Heath must address the following deficiency: Heath had failed to update his address of record. Although Heath provided information on a form complaint, he did not state facts of what took place, when the events took place, where the events took place (such as which particular facility), which defendants were involved, and what each particular defendant did. The statute also requires a prisoner to provide to the Court both an in-forma-pauperis (IFP) application and a certificate of inmate trust account for the six months preceding the date of filing, and Plaintiff has failed to do so. Failure to comply with this order may lead to dismissal for failure to prosecute pursuant to FRCvP 41(b). Deadline to cure the deficiency is 30 days. (Ordered by Judge Reed C.
