ORDER AND NOTICE OF DEFICIENCY: Terrence Terrell Lindsey must address the following deficiency: Although Plaintiff filed a long-form IFP application, he did not complete all sections of the form. See ECF No. 2. For example, he did not complete the section regarding his expected income next month or state how much cash he has. And Plaintiff does not indicate whether he has a spouse that contributes any income. The information sought on the form is necessary for the Court to review and consider when determining Plaintiffs pauper status. Plaintiff must provide complete and accurate answers to all questions on the long-form IFP application..
