ORDER: granting #6 MOTION for Extension of Time. Plaintiff shall, on or before September 17, 2018, pay the filing fee of $400.00 or file a signed Application to Proceed in Forma Pauperis together with a certificate of his inmate trust account. Plaintiff shall also file an Amended Complaint on the 1983 form previously provided by the Clerk. Failure to return the completed amended complaint form will be construed as a voluntary dismissal of the Petitioner's civil rights claims. FILING FEE DEFICIENCY ORDER: Alex Aguirre must address the following deficiency within 20 days from the date of this Order: Pay the filing fee of $400.00 or file with the Clerk a signed Application to Proceed in Forma Pauperis; A certified copy of his inmate trust account statement, A signed copy of the Authorization portion of the Certificate of Inmate Trust Account or a signed copy of any consent form. Plaintiff shall complete and file with the Clerk an Amended Complaint set forth on the form provided by the Court. Failure to file the required documents will result in a denial of informa pauperis status and a dismissal of Plaintiff's complaint for want of prosecution without further notice, unless the filing fee of $400.00 is timely paid. Plaintiff shall promptly notify the Court of any change of address by filing a writtennotice of change of address with the Clerk. Failure to file such notice may result in this case beingdismissed for want of prosecution.
