Plaintiff shall, on or before September 17, 2018, pay the filing fee of $400.00 or file a signed Application to Proceed in Forma Pauperis together with a certificate of his inmate trust account. Plaintiff shall also file an Amended Complaint on the 1983 form previously provided by the Clerk. Failure to return the completed amended complaint form will be construed as a voluntary dismissal of the Petitioner's civil rights claims.
