ORDER TO SHOW CAUSE, NOTICE, AND INSTRUCTIONS TO PARTIES: Respondent is directed to file an answer to Petitioner's petition pursuant to Rule 5 of the Rules Governing Section 2254 Cases in the United States District Courts within sixty (60) days from the date of this Order. No extensions will be granted. ORDER: The Court has considered Petitioner's #5 Application to Proceed in Forma Pauperis and his statement of inmate trust account provided by the institution where he is incarcerated and finds that Petitioner should be granted permission to proceed informa pauperis until further order. The Clerk is directed to create a separate docket entry for the page currently docketed as Doc. 5, Page 3, and label the entry as a supplement to the original petition (Doc. 1 ).Petitioner shall promptly notify the Court of any change of address by filing a written notice of change of address with the Clerk. DEFICIENCY ORDER: It is, therefore, ORDERED that (1) Petitioner shall pay the $5.00 filing fee or file an application to proceed informa pauperis and a certificate of his inmate trust account within thirty (30) days from the date of this Order. The United States District Clerk shall mail to Petitioner an Application to Proceed in Forma Pauperis marked Civil Action Number 5:18-CV-00257-C. (2) Petitioner must also file a supplement to the petition containing his answers to questions 1 - 4. The Clerk shall forward a copy of page 1 of the form habeas petition (doc. #1 ) to Petitioner. Petitioner shall complete the answers to questions 1 - 4, and return the fully completed page for filing with the Clerk within thirty (30) days from the date of this Order. Upon receipt of the completed page, the Clerk shall docket same as a supplement to the original petition.
