Petitioner shall, no later than March 22, 2019, fully complete and return the page to the Clerk for filing, AND pay the filing fee of $5.00, or if unable to pay, file a certificate of his inmate trust account. The failure of Petitioner to respond to this Order shall result in the dismissal of his petition without further notice.
