July 28, 2021 Opinion or Order Filing 5 DEFICIENCY SHOW CAUSE ORDER: On June 11, 2021, Plaintiff was order to pay the $402.00 filing fee or file a signed application to proceed in forma pauperis, a certified cop of his inmate trust account statement and a complete amended complaint, which he has not responded to the Court's order. Thus, the Court finds that Plaintiff shall have 20 days from the date of this order to filed the documents listed or file a sworn affidavit or unsworn declaration made under penalty of perjury pursuant to 28 U.S.C. 1746 and attach copies of any relevant consent forms or other supporting information.
