DEFICIENCY SHOW CAUSE ORDER: On June 11, 2021, Plaintiff was order to pay the $402.00 filing fee or file a signed application to proceed in forma pauperis, a certified cop of his inmate trust account statement and a complete amended complaint, which he has not responded to the Court's order. Thus, the Court finds that Plaintiff shall have 20 days from the date of this order to filed the documents listed or file a sworn affidavit or unsworn declaration made under penalty of perjury pursuant to 28 U.S.C. 1746 and attach copies of any relevant consent forms or other supporting information. ORDER: The Court Orders Livingston must, within 20 days from the date of this Order: pay the filing fee of $402.001 or file with the Clerk a signed application to proceed in forma pauperis with a certified copy of his inmate trust account statement and submit a complete amended complaint on the court- approved form for civil-rights cases. If Livingston fails to file the required documents the Court will deny in forma pauperis status and a dismiss that Plaintiff's complaint for want of prosecution without further notice. Clerk mailed an application to proceed in forma pauperis, and a prisoner civil rights complaint form marked "amended," to Livingston, along with a copy of this Order.
