July 12, 2021 Opinion or Order Filing 4 NOTICE OF DEFICIENCY AND ORDER: Before the Court is a civil rights complaint requesting a name change filed by John Vanous, an inmate confined in the James V. Allred Unit of the Texas Department of Criminal Justice in Iowa Park, Texas. ECF No. #1 . Upon review of the papers and pleadings filed in this action, it appears that Plaintiff has neither paid the Court's filing fee nor submitted a request to proceed in forma pauperis. To proceed in district court, Plaintiff must either pay the $402.00 filing and administrative fees or submit a motion for leave to proceed in forma pauperis. A motion for leave to proceed in forma pauperis must include a statement, certified by an authorized officer of the institution in which Plaintiff is confined, showing Plaintiff's current trust account balance. It is, therefore, ORDERED that Plaintiff shall either pay the $402.00 filing fee or file a motion for leave to proceed in forma pauperis on or before August 11, 2021. A form motion for leave to proceed in forma pauperis and a certified trust account statement are available in the law library of the institution in which Plaintiff is confined.
