Additionally, Plaintiff has neither paid the Court's filing fee nor submitted a request to proceed in forma pauperis. Plaintiff must either pay the $405.00 filing and administrative fees or submit a request to proceed in forma pauperis on the District Court's Application form along with a certified inmate trust account statement.
