ORDER that the complaint be and is conditionally filed pending compliance with the requirements of this Order; pltf's #1 motion for preliminary injunction be and is DENIED; pltf is assessed a filing fee of $350.00 and an administrative fee of $50.00, pursuant to 28 USC sect. 1914(a); pltf's #2 Motion for Leave to Proceed in forma pauperis is conditionally granted to the extent that pltf need not prepay the filing fee at this time; the Clerk request pltf's institution to provide an Inmate Account Report Form on pltf within thirty (30) days of the date of this Order; pltf shall file a particularized and amended complaint within thirty (30) days of the entry of this Order using the standardized form sect. 1983 complaint enclosed with this Order and this amended complaint will serve as the sole complaint in this civil action (see Order for details).
