ORDER. It is ORDERED that within thirty (30) days from the date of this Order, Petitioner must accomplish one of the following actions: 1. Remit $5.00 filing fee. 2. Complete and submit to the Clerk of this Court an "Affidavit in Support of Request to Proceed in Forma Pauperis." It is ORDERED that Petitioner file within 30 days from the date of this Order an original petition on Form AO 241 (01/15). It is further ORDERED that the petition be amended to reflect Harold W. Clarke, Director of the Virginia Department of Corrections, as the proper respondent in this proceeding. The Clerk is DIRECTED to mail a copy of this Order to Petitioner along with an application to Proceed in Forma Pauperis and two copies of Form AO 241 (01/15) with the current case number affixed to each copy.
