It is further ORDERED that along with his amended petition, petitioner shall 1. Remit $5.00 as a filing fee, or 2. Complete and submit an "Affidavit in Support of Request to Proceed in Forma Pauperis." The petition shall remain CONDITIONALLY FILED. The Clerk is DIRECTED to mail another copy of this Order, along with two copies of the above-mentioned forms, with the case number affixed, to the Petitioner at Nottoway Correctional Center. 2892 Schutt Road. P. O. Box 488. Burkeville. VA 23922. Petitioner is DIRECTED to complete and file one copy of each form with the court and to retain the other copy for his records.
