Section 2254," including a proper party defendant and proof of exhaustion. It is further ORDERED that, along with the filing of Petitioner's amended petition, he must 1. Remit $5.00 filing fee. 2. Complete and submit to the Clerk of this Court an "Affidavit in Support of Request to Proceed in Forma Pauperis." The petition shall remain CONDITIONALLY FILED. The Clerk is DIRECTED to mail a copy of this Order, along with two copies each of the above-mentioned forms, with the case number affixed, to the Petitioner at the address on file with the court.
