It is ORDERED that if Plaintiff wishes to proceed in forma pauperis, Plaintiff is ORDERED to sign the enclosed Consent to Collection of Fees form and RETURN IT TO THE COURT within twenty-one (21) days of the date of entry of this Order. If Plaintiff does not wish to proceed in forma pauperis, Plaintiff is ORDERED to pay both the $350.00 statutory filing fee and $52.00 administrative fee (a total of $402.00) within twenty-one (21) days of the date of entry of this Order.
