Filing 2 ORDER that Plaintiff's #1 Motion for Leave to Proceed in forma pauperis is provisionally GRANTED. The plaintiff did not submit the ghosting writing form which is required to be filed with her submission stating she did not receive assistance in preparing her complaint. Therefore, the plaintiff is ORDERED to file with the Court the Ghost Writing Form within 10 days of the date of this Order. Failure to comply with this request will result in the case being dismissed without prejudice. The plaintiff also is DIRECTED to show cause why this case should not be dismissed as duplicative of 3:16cvl47 within 10 days of the date of this Order.
