MEMORANDUM ORDER Assessing Initial Partial Filing Fee. It is ORDERED that within eleven (11) days, Plaintiff must submit an initial partial filing fee of $0.45 or state under penalty of perjury that he does not have sufficient assets to pay such a fee. See 28 U.S.C. 1915(b)(1). 2. Plaintiff remains liable for the entire filing fee of $350.00. The Court will contact Plaintiff's place of incarceration. The institution will then forward to the Court twenty percent (20%) of all deposits into Plaintiff's account until the full $350.00 filing fee is paid. See Order for more details. MEMORANDUM ORDER that this action is CONDITIONALLY docketed; Plaintiff to submit statement as set out herein within 30 days of entry hereof, Plaintiff must submit a statement under oath or under penalty of perjury. The Court is forwarding to Plaintiff an affidavit for compliance with the above procedures. Failure to complete the affidavit in its entirety will result in summary dismissal of the action. Clerk to obtain certified copy of Plaintiff's trust fund account for 6 month period immediately preceding the initiation of this action; Plaintiff to return the Consent to Collection of Fees form within thirty (30) days of entry hereof. Plaintiff asks for a list of civil rights attorneys and copies of is previously dismissed civil action filed in this Court. (ECF No. 1, at 9.) The Court is unable to provide Plaintiff with a list of attorneys. If Plaintiff would like copies of certain documents, he may send a letter to the Clerk requesting copies.
