Filing 3 MEMORANDUM ORDER (Conditionally Filing 28 U.S.C. 2241 Petition and Requiring Petitioner to Complete the Standardized Forms). It is ORDERED that: The petition is CONDITIONALLY FILED pending payment of any assessed filing fee; In the United States District Court for the Eastern District of Virginia, all pro se petitions for writs of habeas corpora must be filed on a set of standardized forms. Accordingly, the Clerk is DIRECTED to mail Petitioner the standardized form for filing a 2241 petition. The Clerk shall WRITE the civil action number for the present action on the form. Petitioner is DIRECTED to complete and return the form to the Court within eleven (11) days of the date of entry hereof. The Clerk shall obtain a certified copy of Petitioners trust fund account for the six (6) month period immediately preceding the initiation of this action. The Clerk is DIRECTED to send Petitioner a Notice & Consent to Proceed Before a United States Magistrate form. Petitioner should only return this form if Petitioner consents to the jurisdiction of the United States Magistrate Judge. If Petitioner consents to the jurisdiction of the Magistrate Judge, Petitioner should return this form within fifteen (15) days of the date of entry hereof. It is so ORDERED.
