It is hereby ORDERED that the action is DISMISSED WITHOUT PREJUDICE, because Plaintiff failed to return a consent to collection of fees form and in forma pauper is affidavit and did not pay the statutory filing fee. Should Plaintiff desire to appeal, a written notice of appeal must be filed with the Clerk of the Court within thirty (30) days of the date of entry hereof.
