ORDER - On July 26, 2021, Plaintiff Derrick M. Allen filed a Motion for Leave to Proceed in forma pauperis with his Complaint attached. (ECF No. 1.) Allen appears qualified to proceed in forma pauperis, therefore, the Motion is provisionally GRANTED. the Court ORDERS Allen to file with the Court the Ghostwriting Form, (a copy of which is enclosed), no later than October 1, 2021. Failure to comply with this request will result in the case being dismissed without prejudice. The Court hereby ORDERS that the Clerk shall provisionally file the proffered Complaint; and it is further ORDERED that, no later than October 1, 2021, Allen shall file an Amended Complaint, with a Ghostwriting Form attached, which outlines in simple and straightforward terms why Allen thinks that he is entitled to relief and why the Court has jurisdiction over his case.
