Filing 3 MEMORANDUM ORDER (Conditionally Filing 28 U.S.C. 2254 Petition andRequiring Petitioner to Complete the Standardized Form). It is ORDERED that: The petition is CONDITIONALLY FILED; In the United States District Court for the Eastern District of Virginia, all pro se petitions for writs of habeas corpus must be filed on a set of standardized forms. The Clerk is DIRECTED to mail Petitioner the standardized form for filing a 2254 petition. The Clerk shall WRITE the civil action number for the present action on the form. Petitioner is DIRECTED to complete, sign under penalty of perjury and return the entire form to the Court within thirty (30) days of the date of entry hereof. Despite submitting only one page of the required standardized form petition, and despite failing to sign his petition under penalty of perjury, Petitioner has moved to amend his petition to change the name of one respondent. Petitioner has not submitted a copy of his proposed amended petition. Plaintiff's request to amend his petition (ECF No. #2 ) is DENIED WITHOUT PREJUDICE; The Clerk is DIRECTED to send Petitioner a Notice & Consent to Proceed Before a United States Magistrate form. Petitioner should only return this form if Petitioner consents to the jurisdiction of the United States Magistrate Judge. If Petitioner consents to the jurisdiction of the Magistrate Judge, Petitioner should return this form within fifteen (15) days of the date of entry hereof. See Order for details. It is so ORDERED.
