Filing 2 ORDER (Denying Application to Proceed In Forma Pauperis, Directing Clerk to Mail Long Form, and Ordering Plaintiff to File Amended Complaint) - This matter comes before the Court on Plaintiff's Application to Proceed in forma pauperis, (ECF No. 1), and proposed Complaint, (ECF No. 1-1). Upon review of Plaintiff's Application, the Court concludes that Plaintiff has submitted the incorrect form. The Court thus DENIES the Application and DIRECTS the Clerk's Office to mail Plaintiff a copy of the Long Form version of the Application. The Court further DIRECTS that Plaintiff shall complete and return the correct Long Form within thirty (30) days of the entry hereof. Failure to do so will result in dismissal of the action. the Court hereby DIRECTS that, within thirty (30) days of the entry of this Order, Plaintiff shall file a particularized amended complaint that outlines in simple and straightforward terms why Plaintiff believes that she is entitled to relief and why the Court has jurisdiction over this case, pursuant to Federal Rule of Civil Procedure 8(a)(1) and (2).
