MEMORANDUM ORDER (Conditionally Docketing Action). It is ORDERED that: This action is CONDITIONALLY docketed. Although Ryan J. Sullivan is listed as the Plaintiff, the action was prepared and signed by Kahlia Smith. Smith cannot litigate this action on Sullivan's behalf. The Clerk is DIRECTED to write the civil action number for this action on the standard form for filing a 42 U.S.C. 1983 action. The Clerk is DIRECTED to mail the form to Plaintiff. Plaintiff must complete and return the form within thirty (30) days of the date of entry hereof. The failure to do so will result in the dismissal of the action. The documents submitted by Ms. Smith will receive no further consideration. Within thirty (30) days from the date of entry hereof, Plaintiff must submit a statement under oath or under penalty of perjury.
