Names as defendants only those individuals he contends are personally responsible for the for the conditions or actions undertaken at Ventress which he believes amount to a violation of his constitutional rights. 2. Specifically describes how the individuals named as defendants personally acted in violation of his constitutional rights. That is, for each individual named as a defendant, Plaintiff must provide facts to show that particular individual's participation or involvement in the claim, as further set out in order. To aid Plaintiff in filing a response to this order, the Clerk is DIRECTED to provide him with a copy of the form used by prisoners to file 1983 complaints, and Plaintiff shall use this form in filing his amended complaint. The court will consider only those claims set forth and defendants named in the amended complaint. The amended complaint shall therefore supersede the original complaint.
