Plaintiff's present custodian is directed to collect an initial partial filing fee of $3.88 and forward that amount along with future monthly payments to the Clerk of Court until a total of $350 has been collected. The Clerk of the Court is directed to send a copy of this Order to the Warden of the Omega Unit, the ADC Trust Fund Centralized Banking Office, and the ADC Compliance Office. The Clerk is directed to send Plaintiff a blank 1983 complaint form.
