ORDER: Plaintiff's application for leave to proceed in forma pauperis ("IFP") #3 is denied without prejudice. If Plaintiff wants to proceed with this lawsuit, he must either: (1) submit a renewed application that is accompanied by a certified copy of Plaintiff's trust fund statements showing activity in his account or accounts for the entire six-month period immediately preceding his initiation of this action; or (2) pay the full statutory filing fee of $400.00. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. Failure to comply with this order by 12/9/19, will result in summary dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order. Plaintiff's motion for attorney representation #4 , which indicates that he has made no effort to obtain counsel on his own, is denied without prejudice to renewal after he has made a reasonable effort to secure his own counsel.
