ORDER: Plaintiff Randy Norwood's application for leave to proceed in forma pauperis [Dkt. 3] is granted. The filing fee is waived. Norwood may proceed on his complaint consistent with this order [Dkt. 1]. The Court directs the Clerk to: (1) file Norwood's complaint [Dkt. 1], (2) issue summonses for service of the complaint on Defendants Wexford Health Service, Inc., Dr. West, Ms. Czerniak, and Sheriff Hain, (3) terminate all other named Defendants, (4) send Norwood four USM-285 forms, filing instructions, and a copy of this order, and (5) send a copy of this order the U.S. Marshals Service. The Marshal is appointed to effect service and is authorized to send a request for waiver of service in the manner prescribed by Fed. R. Civ. P. 4(d) before attempting personal service. The U.S. Marshal will not effect service until the USM- 285 service forms are returned. Norwood must complete and return the USM-285 forms by May 16, 2023. Failure to do so may result in dismissal of any unserved Defendant, as well as dismissal of this case in its entirety. Norwood also must promptly submit a change-of-address if his address changes while this case remains pending. If Norwood fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute.
