ORDER re #1 MOTION for Leave to Proceed in forma pauperis filed by Samuel Bernard Jemison Cunningham: Plaintiff filed a motion to proceed in forma pauperis, it does not comply with the requirements because it does not contain a certified copy of his prison account statement. Accordingly, plaintiff is given thirty days from the date of this order to either pay the complete $402 filing fee or file an amended motion to proceed in forma pauperis which complies with the rules set out above. Additionally, the complaint (Doc. 1-1) is deficient in a number of ways. Plaintiff is given thirty days from the date of this order to file an amended complaint using the standard Section 1983 form and setting out his claims as is required by the Federal Rules of Civil Procedure. If plaintiff fails to file an amended complaint within thirty days, this case will be dismissed with no further order of the Court for failure to state a claim.
