ORDER: 1) Parker's #9 Motion for Leave to Proceed in forma pauperis is GRANTED. prisoner-plaintiff to pay the $350 as set forth below; 2) w/i 28 days Parker must pay $49.47 to the Clerk of Courts as an initial fee; 3) The clerk shall open an account i Parker's name. The clerk shall complete a Notice of Payment Form as set out and shall serve a copy of this Order and the Notice of Payment Form upon the Jailer of the institution in which Parker is currently confined; 4) Each month Parker's custodian shall pay an amount equal to 20% of his income the preceding month but only if that amount exceeds $10. The custodian shall continue monthly payments until the $350 is pain in full; 5) Parker must immediately advise the Clerk's office of any change in his current mailing address. ORDER: 1) Parker's #2 Motion for Leave to Proceed in forma pauperis is DENIED WITHOUT PREJUDICE; 2) The clerk shall mark the assigned case number on a EDKY Form 520 and send it to Parker. The Clerk shall also send Parker (1) Form AO-240 and (2) a EDKY Form 523; 3) Should Parker wish to pursue this claim he must complete and file the complaint IN ITS ENTIRETY describing FACTS, specifically identifying the people, dates, places and actions relevant to his claims; 4) In addition, Parker must either pay the $400 OR (1) have the Certificate of Inmate Account certified, (2) complete the Affidavit of Assets/IFP Application and (3) file both with the Court; 5) Parker must take these WITHIN 30 DAYS or this matter will be dismissed; 6) The Clerk shall update the docket to reflect that Parker's current address.
