ORDER: Accordingly, it is hereby ORDERED as follows:1. The Clerk shall mark the assigned case number on a Sixth Circuit Form 4 and a Certificate of Inmate Account [EDKY Form 523] and forward both to Askew. 2. Askew must either pay the $505.00 appellate filing fee or (1) have prison staff certify the Certificate of Inmate Account [EDKY Form 523], (2) complete the Sixth Circuit Form 4, and (3) file both of these documents with this Court (the District Court, not the United States Court of Appeals for the Sixth Circuit). 3. Askew must take each of these steps within twenty-eight (28) days or the Sixth Circuit will dismiss the appeal for failure to prosecute. 4. ORDER re #1 Petition for Writ of Habeas Corpus filed by Anthony Askew: Accordingly, it is hereby ORDERED as follows: 1. To the extent that Askew's initial pleading [R. 1] requests that payment of the $5.00 filing fee be waived, that request is DENIED without prejudice. 2. To the extent that Askew's initial pleading [R. #1 ] requests expedited preliminary injunctive relief, that request is DENIED. 3. The Clerk shall send Askew a Petition for Writ of Habeas Corpus [EDKY Form 521], an Affidavit of Assets / In Forma Pauperis Application [Form AO-240] and a Certificate of Inmate Account [EDKY Form 523]. 4. Within 30 days, Askew must complete and file the habeas petition form, providing all of the facts relevant to his claims and stating the legal grounds for the relief he seeks. 5. Within 30 days, Askew must also either pay the $5.00 filing fee or (1) have the Certificate of Inmate Account [EDKY Form 523] certified by prison staff, (2) complete the Affidavit of Assets/In Forma Pauperis Application [Form AO-240], and (3) file both documents with the Court. 6.
