Filing 14 ORDER: (1) Johnson's motion for leave to proceed in forma pauperis on appeal (Doc. #12 ) is GRANTED. Johnson must pay the $505.00 appellate filing fee in the manner set forth in this Order. (2) Within 30 days from the entry of this Order, Johnson must pay $25.33 to theClerk of this Court as an initial partial appellate filing fee. (3) The Clerk of this Court shall open an account in Johnsons name for receipt of the appellate filing fee. The Clerk shall complete a Notice of Payment Form (E.D. Ky. 525 Form) with (a) Johnsons name, (b) his inmate registration number, and (c) this case number. The Clerk shall then send a copy of this Order and the Notice of Payment Form to the Warden of the institution in which Johnson is currently confined; (4) Each month, Johnson's custodian shall send the Clerk of the Court a payment in an amount equal to 20% of Johnson's income for the preceding month out of his inmate trust fund account. (5) The Clerk of this Court shall send a copy of this Order to the Clerk of the United States Court of Appeals for the Sixth Circuit. Signed by Judge David L. Bunning on 10/25/2022.
