ORDER: 1. Clerk identify Pla as Anthony Heath Baker and list Heath Anthony Baker, Ashley Anthony Baker, and Ashley Heath Baker as alternative designations. 2. Court GRANTS Baker's DE #3 motion to proceed in forma pauperis. 3. Because Baker granted pauper status, Court WAIVES $52 administrative fee. 4. Court DIRECTS Clerk open an account for receipt of filing fee. Court further DIRECTS Clerk to complete (Form EDKY 525) and send Notice and Order to warden of institution Baker is confined. 5. Court ORDERS Baker's custodian to deduct $1.56 from inmate account and send sum to Clerk nlt May 19, 2023, as an initial partial filing fee. 6. Court further ORDERS Baker's custodian to send Clerk a payment each month, only if account balance exceeds $10. The payment should equal 20% of amount deposited during preceding month.
