Rather, the original QC notice pointed out that the named Defendant in the caption, differed from how it appeared in the complaint (specifically "Frederick J. Corder doing business as Corder Pediatrics Medicine" is Defendant in caption of complaint and "Corder Pediatrics Medicine" (alone) is the named Defendant in body of complaint). The clerk can not make the determination of name on summons when it does not appear exactly the same in the complaint. The praecipe caption differs from the original complaint caption as well.
