Filing 18 MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that plaintiff's motions to proceed in forma pauperis 10 and 17 are GRANTED. IT IS FURTHER ORDERED that the plaintiff must pay an initial filing fee of $1.00 within twenty-one (21) days of th e date of this Order. Plaintiff is instructed to make his remittance payable to "Clerk, United States District Court," and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) that the remittanc e is for an original proceeding (Initial Partial Filing Fee due by 10/26/2016). IT IS FURTHER ORDERED that the Clerk of Court shall provide to plaintiff, along with a copy of this Order, a blank complaint form for the filing of a civil complaint. I T IS FURTHER ORDERED that plaintiff shall file a second amended complaint, in accordance with the instructions set forth above, no later than twenty-one (21) days from the date of this Order (Amended/Supplemental Pleadings due by 10/26/2016). IT IS FURTHER ORDERED that if plaintiff fails to timely file a second amended complaint, or fails to comply with the instructions set forth above relating to the filing of the second amended complaint, the Court WILL dismiss this action without prejudice.
