DEFICIENCY ORDER - re #1 Complaint, filed by Christopher G. Ludwick. Plaintiff is ORDERED to submit a legible, signed complaint within thirty days of this Deficiency Order. Second, Plaintiff is ORDERED to complete and return a Civil Cover Sheet with his new complaint. Third, Plaintiff has not paid the filing fees to institute this action. The fee for a civil action is $402 (a $350 filing fee and a $52 administrative fee). 28 U.S.C. 1914(a). If Plaintiff does not have sufficient funds to pay these fees in full at this time, he may submit an Application to proceed in forma pauperis and ask the Court to allow him to pay the filing fee over time. 28 U.S.C. 1915(b). Plaintiff is ORDERED to either pay $402 to proceed in this case, or to submit an in forma pauperis Application with supporting documents within thirty days of the date of this Deficiency Order. Finally, Plaintiff is advised that he must keep this Court informed of his current address and promptly file a Notice of New Address if he is released or transferred..
