SECOND DEFICIENCY ORDER - Petitioner is again ORDERED to either pay the $5.00 filing fee or submit an application to proceed in forma pauperis on the appropriate form WITHIN THIRTY DAYS of the date of this Second Deficiency Order. He is also ORDERED to submit to the Court an Amended Petition, using the appropriate standard form, setting out his grounds for relief and the other required information. The Amended Petition must name a proper respondent over which this Court would have jurisdiction and upon which this Court could order service. The Amended Petition must be submitted WITHIN THIRTY DAYS of the date of this Second Deficiency Order. Petitioner is ADVISED that failure to comply with this Second Deficiency Order will result in the dismissal of this action for want of prosecution and/or failure to comply with the Court's orders.Finally, Petitioner is again DIRECTED not to submit further documents in this case with a decorative border around the top margin, as it obscures the Court's filing information.
