July 22, 2024 Opinion or Order Filing 3 NOTICE OF DEFICIENCY and ORDER - IT IS THEREFORE ORDERED: (1) Plaintiff VanHorn must submit, within 30 days of the date of this Order, a certified copy of his prison trust fund account statement (or institutional equivalent) signed by the institutional cashier or other institutional employee authorized to sign such forms for the six-month period prior to the filing of the complaint. (2) Plaintiffs Cook, Knott, Bared, and Severance must each, within 30 days of the date of this Order, submit to the Court either: (a.) An in forma pauperis motion/application and certified copy of his prison trust fund account statement (or institutional equivalent) signed by the institutional cashier or other institutional employee authorized to sign such forms for the six-month period prior to the filing of the complaint, or (b.) His respective portion of the $405.00 filing fee. (3) The CLERK OF COURT is DIRECTED to provide Cook, Knott, Bared, and Severance with an Application and Affidavit By Incarcerated Person to Proceed Without Prepayment of Fees form. (4) If any plaintiff fails to comply with this Order, they will be dismissed from this action as a plaintiff for want of prosecution. If any plaintiff's case is dismissed for failure to comply with this Order, their case will not be reinstated to the Court's active docket. (5) Plaintiffs are also informed that any motion for extension of time to comply with this Order must be filed within thirty (30) days of the date of this Order. Signed by Magistrate Judge Karen L.
