ORDER granting #1 Motion for Leave to Proceed in forma pauperis. Having reviewed the Complaint, and without the benefit of briefing by the parties, the Court concludes that Plaintiff's claims against Defendant may proceed at this juncture. Plaintiff has requested the issuance of summons and filed service forms. (Docs. #1 -3, -4). However, the USM 285 form Plaintiff filed is deficient. (Doc. #1 -4). Specifically, Plaintiff failed to sign, date, and provide his telephone number as instructed halfway down the form. Plaintiff is ORDERED to complete these lines and resubmit the form within the next ten (10) days. The Clerk is DIRECTED to mail Plaintiff back the form he filed to facilitate this.
