MEMORANDUM and ORDER FOR ADMINISTRATIVE CLOSING. This action may not proceed unless the Plaintiff either, 1) tenders to the "Clerk, U.S. District Court" the amount of $402.00; or, 2) files a properly completed application to proceed in forma pauperis, including an affidavit that includes a statement of all assets such prisoner possesses that the person is unable to pay such fees or give security therefor. 28 U.S.C. 1915(a)(1). Additionally, pursuant to Federal Rule of Civil Procedure 3, a federal court civil lawsuit is commenced by filing a complaint with the Court. Therefore, Plaintiff must submit a properly completed Complaint using the Courts approved form, which Plaintiff must sign and date.
